I've found that by keeping track of everything, no matter how small, helps me make sure that I'm accurate with the information I give to my doctors, SSDI, and any other professionals I see for my condition. Here are some simple forms I've used to keep track. I highly recommend getting a binder or folder to keep them all in (you'd be surprised how fast they will fill up!). I'm just now designing and carrying binders in my Zazzle shop (link to the left) if you'd like to purchase a binder just for your your worksheets. The worksheets are in PDF format. If your computer cannot support a PDF document, please contact me and I will be more than happy to email you a format that works better for you.

Family Resource Network of Central Florida
The Family Resource Network of Central Florida is a community-based network for children and teens with diabetes and their families. Teen Diabetes Support Group Led by Cindy Fedor, MSW
The Teen Support Group meets the first Monday of most months (call for schedule) at the Easter Seals Peoria Center, 507 East Armstrong AvenuePeoria, Illinois 61603.
Teen Social and snack time will be at the start of each group meeting from 6-6:15 pm. During the teen social time, parents are welcome to meet in a separate room and then join the teens at 6:15 pm.
